Approval to Work Onsite during NSW Health Stay at Home Order Saturday 26 June until xxxxxx

This letter is to confirm that you have been assessed by management as an essential worker and permission is granted for you to undertake your specified work activities at the XXXXX premises, while the “work from home” directive is in place.
Venues of concern: Check NSW Health site before leaving home and ensure you are not under a directive from having attended a site under investigation. Please monitor these lists, follow NSW Health rules for testing and isolation if you have been to any of the venues.
You will be required to use the government QR scan code and sign-in/sign-out at reception each day at the front reception. A Site Controller will be on site each weekday (listed at reception including mobile phone number), who
will conduct a walk-through of the buildings each day and be available as a back-up person.
You are reminded not to attend xxxxx if you feel unwell, are awaiting test results for COVID19,
have any cold and flu symptoms (mild or severe), or have been in contact with someone with a
confirmed or suspected case of COVID19.
